Antipyretika bei Intensivpatienten

Zusammenfassung

Hintergrund

Antipyretika gehören zu den am häufigsten verordneten Medikamenten in deutschen Kliniken. Trotz dieser weiten Verbreitung sind Indikation, Effekte und Nebenwirkungen bei der Behandlung von Intensivpatienten noch nicht zufriedenstellend abgeklärt.

Ziel der Arbeit

Zielsetzung ist die Zusammenschau und kritische Diskussion aktueller Studienergebnisse zur Anwendung von Antipyretika bei Intensivpatienten. Fragliche Effekte in Hinblick auf Fiebersenkung und Letalität sollen beleuchtet werden.

Material und Methoden

Es erfolgte eine selektive Literaturrecherche in der Datenbank PubMed sowie die Sichtung der Literaturverzeichnisse aller als relevant erachteten Arbeiten.

Ergebnisse

Es gibt aktuell nur wenige Studien zum gezielten Einsatz von Antipyretika bei Intensivpatienten, die sich aufgrund unterschiedlicher Designs, Protokolle und Endpunkte nur schwer systematisch vergleichen lassen. Die absolute Fiebersenkung war mit 0,3 °C sehr gering und zeigte bei Sepsis sogar nachteilige Effekte auf die 28-Tage-Sterblichkeit. Mit ASS behandelte Patienten hatten eine reduzierte Sterblichkeit in retrospektiven Studien.

Diskussion

Der Nutzen einer antipyretischen Behandlung von Intensivpatienten unter Beachtung harter Endpunkte wie der Letalität bleibt unklar. Weitere kontrollierte Studien mit vergleichbaren Protokollen und repräsentativen Stichproben sind notwendig, um die Basis für wegweisende Behandlungsempfehlungen zu schaffen.
