Zusammenfassung

Hintergrund

Die Prävalenz der nichtalkoholische Fettlebererkrankung (NAFLD) nimmt stetig zu. Etwa 25 % der Weltbevölkerung und mehr als die Hälfte der Patienten mit Diabetes mellitus Typ 2 oder Adipositas sind von ihr betroffen.

Fragestellung

Die Komplikationen und Folgezuständen der NAFLD werden dargestellt.

Material und Methode

Aktuelle Expertenempfehlungen, Leitlinien und wichtige Studien zur NAFLD wurden gesichtet und ausgewertet.

Ergebnisse

Die NAFLD gliedert sich in die histologisch definierten Entitäten nichtalkoholische Fettleber (NAFL) und nichtalkoholische Steatohepatitis (NASH). Dabei ist die Mortalität von Patienten mit bei der Diagnosestellung vorliegender NASH erhöht. Diese kann zur Leberzirrhose mit Komplikationen des portalen Hypertonus und der Leberinsuffizienz führen. Das hepatozelluläre Karzinom als Spätkomplikation der NASH kann sich auch in einer nichtzirrhotischen Leber entwickeln. Die leberassoziierte Mortalität steht an dritter Stelle der Todesursachen bei NAFLD. Diese stellt außerdem einen Risikofaktor für kardiovaskuläre Erkrankungen und extrahepatische Malignome dar, die an erster und zweiter Stelle der Todesursachen stehen. Der Grad der Leberfibrose ist der wichtigste Prädiktor für die Gesamtmortalität bei Patienten mit NAFLD, wobei Typ-2-Diabetes und arterieller Hypertonus Risikofaktoren für deren Progression sind. Umgekehrt haben NAFLD-Patienten ein erhöhtes Risiko, an Typ-2-Diabetes zu erkranken. Basistherapie ist eine Lebensstilintervention mit dem Ziel der Gewichtsnormalisierung durch diätetische Maßnahmen und gesteigerte körperliche Aktivität.

Schlussfolgerungen

Die NAFLD ist eine der häufigsten Lebererkrankungen und mit einer relevanten hepatischen und extrahepatischen Morbidität und Mortalität assoziiert.
