Zusammenfassung

Hintergrund

Das Neugeborenenscreening ist die bei Weitem erfolgreichste Maßnahme zur Sekundärprävention gesundheitlicher Beeinträchtigungen. Aktuelle Fortschritte in Diagnostik und Therapie legen eine Erweiterung um weitere Zielkrankheiten in Deutschland nahe.

Ziel der Arbeit

Dieser Leitthemenbeitrag gibt einen Überblick über bisherige Strukturdefizite, mögliche zukünftige Zielkrankheiten des Neugeborenenscreenings und den aktuellen Stand von Pilotprojekten zu zusätzlichen Screeningkonzepten in Deutschland.

Material und Methoden

Auf Grundlage der bisherigen Datenlage werden Empfehlungen zu Verbesserungen von Abläufen und Strukturen sowie zur Aufnahme neuer Zielkrankheiten entwickelt.

Ergebnisse

In Deutschland bewahrt das Screening auf angeborene Stoffwechsel- und Hormonstörungen jährlich mehr als 500 Kinder vor schwerer Behinderung oder gar Tod. Nicht alle Bundesländer verfügen bisher über „Tracking“-Zentralen, um kontrollbedürftige Befunde konsequent nachzuverfolgen. Im Jahr 2016 wurde die Mukoviszidose (zystische Fibrose, CF) als 15. Zielkrankheit aufgenommen. Ein Screening auf zusätzliche angeborene Stoffwechselkrankheiten unter Anwendung von „Second-tier“-Strategien wird derzeit in Pilotstudien untersucht. Weitere Kandidaten für eine Erweiterung des deutschen Screeningpanels sind Sichelzellkrankheiten und schwere kombinierte Immundefizienzen (SCID).

Schlussfolgerung

Vor Einführung jeder einzelnen möglichen neuen Zielkrankheit sind differenzierte Evaluationen erforderlich. Ziel ist, dass in Zukunft noch wesentlich mehr Kinder vom Neugeborenenscreening profitieren.
