Quadrizepssehnenentnahme zur VKB-Rekonstruktion

Zusammenfassung

Die Quadrizepssehne als Transplantat für VKB(vorderes Kreuzband)-Rekonstruktionen gelangt in den letzten Jahren mehr in den Fokus. Vorteile des Quadrizepstransplantates sind die geringere Entnahmemorbidität sowie sehr gute Stabilitäts- und Funktionsergebnisse im Vergleich zu den Hamstring- oder Patellasehnentransplantaten. Das Ziel dieser technischen Beschreibung ist es, die einzelnen Schritte der Entnahme eines rein weichteiligen, gedoppelten Quadrizepssehnentransplantates aufzuzeigen. Das Quadrizepssehnentransplantat wird über einen 2–3 cm großen Längszugang im Bereich des proximalen Patellapols entnommen. Der Sehnenstreifen wird bis kurz vor dem M. rectus femoris präpariert. Um mehr Länge zu gewinnen, wird nach distal das Periost der Patella mitpräpariert. So wird ein ausreichend langes Transplantat von 14 cm generiert, welches doppelt durch ein fixiertes oder justierbares Button-System gelegt werden kann. Vorteile dieser Entnahmetechnik sind eine technisch einfache Transplantatpräparation sowie ein nicht vollschichtiger Entnahmedefekt, welcher potenziell besser funktionell kompensiert werden kann.
