Stand der Diagnostik und Therapie der inflammatorischen Kardiomyopathie

Zusammenfassung

Unter einer entzündlichen Kardiomyopathie wird ein durch infektiöse Erreger sowie nicht infektiöse Noxen, toxische Substanzen oder Medikamente hervorgerufener, akut oder chronisch-rezidivierend im Herzmuskel verlaufender Entzündungsprozess verstanden. Oftmals handelt es sich um ein Folgestadium einer mehr oder weniger abgeheilten Virusmyokarditis, das sich im Rahmen der primären Immunantwort gegen die auslösende Noxe entwickelt hat. Der Entzündungsprozess kann spontan ausheilen, aber auch Ursache einer diffusen Myokarderkrankung mit schwerer Herzinsuffizienz und langfristig ungünstiger Prognose sein. Die klinisch-apparative Diagnostik kann zusammen mit der Anamnese infrage kommende Differenzialdiagnosen vermuten und Hinweise über das Ausmaß der myokardialen Funktionseinschränkung liefern. Eine definitive Diagnose der häufig vorliegenden viral-entzündlichen Genese, die die Voraussetzung für eine gezielte immunsuppressive oder antivirale Therapie darstellt, ist aber nur durch histologische, immunhistologische und molekularbiologische Untersuchungen an Myokardbiopsien möglich.
