Stellenwert der Kombination von Biologika mit Methotrexat in der Behandlung der Psoriasisarthritis – Fragen bleiben offen

Zusammenfassung

Hintergrund

Die Kombination mit Methotrexat (MTX) verbessert die therapeutische Wirkung von Biologikatherapien in der Behandlung der rheumatoiden Arthritis (RA). Der Einfluss von MTX auf eine Biologikatherapie in der Behandlung der Psoriasisarthritis (PsA) ist bislang jedoch nicht vollständig geklärt, da Daten aus randomisierten klinischen Studien fehlen. Bekannt ist bisher nur, dass Patienten mit PsA bei vorherigem unzureichenden Ansprechen auf nichtsteroidale Antirheumatika oder MTX gleich gut auf eine folgende Biologikatherapie ansprechen.

Fragestellung

Unklar ist, ob MTX-naive Patienten durch die Kombination aus MTX und Biologikum eine stärkere Verbesserung der Krankheitsaktivität erfahren als bei alleiniger Biologikatherapie und ob Patienten unter MTX-Medikation, bei denen eine Biologikatherapie zusätzlich begonnen wird, sich klinisch verschlechtern, wenn MTX abgesetzt wird.

Material und Methoden

Die aktuelle Datenlage und deren Limitationen werden dargestellt. Ferner wird eine forschungsinitiierte multizentrische randomisierte klinische Studie bei Patienten mit aktiver PsA vorgestellt, die den Einfluss einer placebokontrollierten Kombinationstherapie aus MTX und dem Interleukin-12/23-Inhibitor Ustekinumab (UST) untersucht (MUST-Studie), um die bestehende Evidenzlücke zu schließen.

Ergebnisse

Das primäre Ziel der MUST-Studie ist, die Nichtunterlegenheit einer UST-Monotherapie im Vergleich zur MTX/UST-Kombinationstherapie, gemessen am erreichten mittleren DAS28 in Woche 24, nachzuweisen. Eingeschlossen wurden bisher 77 von 196 geplanten Patienten. Die Rekrutierung ist weiterhin offen.

Schlussfolgerung

Die MUST-Studie bietet die Gelegenheit, in einem kontrollierten Studiendesign zu untersuchen, ob die Ergänzung bzw. die Beibehaltung von MTX zu einer UST-Therapie einen Benefit für Patienten mit PsA darstellt.
