Stellenwert von Vasopressin im septischen Schock

Zusammenfassung

Das Ziel der Kreislauftherapie im septischen Schock ist die Wiederherstellung einer adäquaten Gewebeperfusion. Das wachsende Verständnis über die Störungen der Mikrozirkulation in der Sepsis führte dazu, dass die Schockbehandlung zunehmend auf Indikatoren der Gewebeperfusion und in geringerem Ausmaß als zuvor auf arteriellen und zentralvenösen Druckverhältnissen basiert. Mit diesem Wandel hat sich der Stellenwert der Vasokonstriktoren im septischen Schock verändert. Diese Übersichtsarbeit fasst die pathophysiologischen und therapeutischen Eckpunkte der septischen Schockbehandlung zusammen und evaluiert die Evidenz zum Einsatz von Vasopressin als nichtadrenergem Vasokonstriktor im septischen Schock kritisch. Anhand der vorliegenden Studienergebnisse scheint Vasopressin bei erwachsenen Patienten mit moderatem septischem Schock (Noradrenalinbedarf < 15 μg/min) und fehlenden Zeichen einer systemischen Hypoperfusion (z. B. normale Laktatkonzentrationen) potenziell hilfreich zu sein. Eine Vasopressininfusion mit dem alleinigen Ziel der Blutdruckanhebung trotz Zeichen einer systemischen Hypoperfusion ist gefährlich und kann zu einer kritischen Verschlechterung der Gewebedurchblutung führen.
