Medizinische Indikationsstellung und therapeutisches Vorgehen

Zusammenfassung

Die Vorgehensweisen im klinischen Umgang mit der Dehydratation Sterbender wird in Fachkreisen, ebenso wie in der Allgemeinbevölkerung, weiter kontrovers diskutiert. In diesem Beitrag wird die Bedeutung der therapeutischen Indikationsstellung vor dem Hintergrund fehlender Evidenz hervorgehoben. Möglichkeiten der nichtinvasiven Objektivierung der Dehydratation sowie der Beurteilung der Reversibilität der Symptomatik werden aufgezeigt, und der Stellenwert des Monitorings wird erörtert. Die pathophysiologischen Grundlagen reversibler Störungen des Flüssigkeitshaushalts werden beschrieben und von der irreversiblen, terminalen Dehydratation abgegrenzt. Ist eine klinisch assistierte Hydrierung („clinically assisted hydration“, CAH) indiziert, muss der Hydrationsstatus individuell sowie im Voraus beurteilt werden, und alle therapeutischen Maßnahmen sind ständig an die Ergebnisse nichtinvasiver Überwachungsverfahren anzupassen.
