Wieder als konsolidierende Behandlung von Bedeutung?

Zusammenfassung

Die Strahlentherapie (RT) hat bei der Primärtherapie des Ovarialkarzinoms keine Bedeutung, weder als perkutane Ganzabdomenbestrahlung noch als intraperitoneale Therapie mit 32Phosphor, obwohl die Heilungsraten nach Operation und Chemotherapie unbefriedigend sind. Die Zellen des epithelialen Ovarialkarzinoms sind zwar radiosensibel, aber bei seiner Therapie ist der gesamte abdomino-pelvine Raum Zielvolumen der RT, und dessen Organe haben eine sehr eingeschränkte Strahlenverträglichkeit. Die tolerierte RT verbesserte in vergleichenden Studien die Ergebnisse der postoperativen Chemotherapie nicht, und höhere Strahlendosen verursachten Spätreaktionen Grad III/IV. Der Nutzen der palliativen RT ist nicht bestritten. Durch den technischen Fortschritt wurden neue Applikationstechniken der Bestrahlung entwickelt, auch für große Räume, die toleranzgefährdete Organe von der Strahlung aussparen, kritische Volumen konformierend bestrahlen und Patienten- und Organbewegungen berücksichtigen („intensity modulated radiation therapy“, IMRT). Möglicherweise könnte die RT wieder als erfolgreiche konsolidierende Therapie eingesetzt werden.
