A 74-year-old man on warfarin and indefinite antimicrobial therapy for a metallic mitral valve and chronically infected endoluminal abdominal aortic graft presented with a 1-week history of melaena. The patient was haemodynamically stable with anaemia (63 g/L), raised urea (15 mmol/L), supratherapeutic INR (4.4) and raised inflammatory markers (white cell count 18 × 109 /L; C-reactive protein 97 mg/L). Warfarin was reversed and haemoglobin incremented appropriately with blood transfusions. …
