To the Editor—We are interested in Pelta,
et al.’s paper on subcutaneous fissurotomy.
1 We were surprised by the word, “novel,” in the title implying something new or original. Fissure incision or excision without a procedure on the internal sphincter is an old operation and was championed by Ashton in his 1854 text, Hirschman in his 1914 text, and by Gant in his 1923 treatise.

4 Most significantly the authors’ technique, described as “…subcutaneous fissurotomy…the tract was…surgically laid open along its entire length. The residual sentinel tag and skin edges were excised…” closely follows the technique described by Nesselrod in his 1964 textbook.
5 Thus, the statement in Ecclesiastes, “…there is nothing new under the sun,” seems especially relevant to this situation.
