Subretinale Chirurgie bei Massenblutung

Zusammenfassung

Die subretinale Massenblutung im Rahmen einer exsudativen altersabhängigen Makuladegeneration bleibt eine Domäne der submakulären Chirurgie. Während kleinere Blutungen nur aus dem Makulabereich verdrängt und nicht entfernt werden müssen, sind ausgedehnte submakuläre Blutungen weiterhin eine chirurgische Herausforderung mit potenziell schlechtem Visuserfolg. In dieser Übersicht werden die verschiedenen chirurgischen Techniken zur Entfernung von großflächigen Massenblutungen vorgestellt und anhand der vorliegenden Literatur bewertet. Invasive Verfahren wie die einfache Membranextraktion, die Makulatranslokation oder die RPE-Aderhautpatch-Translokation können bei einer Massenblutung zwar prinzipiell eine gute Sehverbesserung bewirken, bei Auftreten von Komplikationen ist die funktionelle Prognose jedoch nicht gut. Eine deutlich geringere Komplikationsrate und bessere Visusergebnisse zeigt hingegen die 2-zeitige Strategie mit rekombinanter Tissue-Plasminogen-Aktivator (rTPA)-assistierter Lyse und anschließender subretinaler Drainage der verflüssigten Blutung über eine kleine Retinotomie.
