The authors of the FINNALI study, published in this issue of Intensive Care Medicine [
1], are to be congratulated for a huge task: to describe the incidence and other epidemiological features of acute respiratory failure (ARF) in a whole country, Finland. As ARF is a frequent cause of admission to the ICU, it is surprising how few studies aim at characterising it [
