85. Therapie des distalen Choledochus-Karzinoms: Evidenz der operativen Therapie

Zusammenfassung

Gallengangs- oder Cholangiokarzinome umfassen Tumoren verschiedener anatomischer Lokalisationen im Bereich des Gallengangsbaums. Extrahepatische Gallengangskarzinome werden zunehmend differenziert betrachtet. Seit der 7. Auflage der TNM-Klassifikation maligner Tumoren werden Karzinome der distalen extrahepatischen Gallengänge und der perihilären Gallengänge erstmalig getrennt klassifiziert. Chirurgisch erscheint bei den Karzinomen der distalen extrahepatischen Gallengänge zusätzlich auch eine Abgrenzung des distalen Choledochus-Karzinoms und des mittleren Choledochus-Karzinoms sinnvoll. Die partielle Pankreatoduodenektomie ist das Standardverfahren in der chirurgischen Therapie des distalen Choledochus-Karzinoms, während alleinige Gallengangsresektionen das Ziel einer R0-Resektion nur zu etwa 10 % erreichen und damit keine sinnvolle Verfahrenswahl darstellen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
