Zusammenfassung

Die Therapie des diabetischen Fußes kann nur interdisziplinär und fachübergreifend gelingen. Im Rahmen der konservativen Therapie stellt das TIME-Konzept ein erfolgreiches Modell dar. Neben der konservativen Lokaltherapie und Wundbehandlung und der sachgerechten Infektionstherapie kommt der Optimierung der Gefäßsituation und somit der Sauerstoff- und Nährstoffversorgung im Wundareal ein besonderer Stellenwert zu. Die Gefäßrekonstruktion erfolgt idealerweise minimal-invasiv radiologisch interventionell oder bei ausgeprägten Befunden durch entsprechende gefäßchirurgische Maßnahmen. Bei Notwendigkeit einer Amputation sollte eine Minor- (unterhalb des Knöchels) oder auch Syme-Amputation angestrebt werden, um eine möglichst große Mobilität des Patienten zu erhalten. Sofern nötig müssen selbstverständlich auch Major-Amputationen durchgeführt werden. Bei größeren und komplizierten Gewebsdefekten ist zusätzlich die Expertise des plastisch-rekonstruktiv tätigen Chirurgen gefragt.

Mesenchymale Stammzellen praktisch nutzlos gegen Gonarthrose

Die Idee, mit mesenchymalen Stammzellen arthrotische Kniegelenke zu regenerieren, klingt vielversprechend. Entsprechend zahlreich sind die Angebote dafür. Die Therapie ist allerdings nicht ganz billig – und vermutlich ohne großen Effekt, wie eine Metaanalyse einschlägiger Studien zeigt.
