Therapieoptionen bei Psoriasis aus Sicht der Dermatologie

Zusammenfassung

Die Psoriasis wurde viele Jahre fälschlicherweise als unangenehme, aber im Grunde simple und harmlose chronische Hautkrankheit betrachtet. Viele wissenschaftliche Untersuchungen der letzten Jahre haben gezeigt, dass die Psoriasis eine schwere Multiorgankrankheit ist mit definierten Auswirkungen auf das Herz-Kreislauf-System und verschiedene endokrine Funktionen. Folge ist eine deutlich reduzierte Lebenserwartung. Neben diesen klar messbaren physiologischen Beeinträchtigungen führt die enorme psychische Beeinträchtigung dieser PatientInnen aber auch zu einer beträchtlichen Verminderung der Lebensqualität.
Bis vor einigen Jahren wurden schwere Erkrankungen systemisch mit Methotrexat, Cyclosporin A, Retinoiden und selten auch mit Fumarsäureestern oder verschiedenen photobiologischen Methoden recht gut behandelt. Mit der Einführung von Biologika hat die Behandlung der Psoriasis (Haut und Gelenke) eine völlig neue Dimension erreicht – die Therapie wurde revolutioniert. Selbst schwerste, jahrzehntelange Krankheiten werden bei adäquater Biologika-Therapie innerhalb weniger Wochen erscheinungsfrei.
