Zusammenfassung

Die fokale HIFU-Therapie stellt einen experimentellen kurativen Therapieansatz dar und kommt für Patienten mit einem niedrig bis früh-intermediären Risikoprostatakarzinom oder einem Rezidiv nach externer Radiatio in Betracht. Im Vordergrund für die Therapieplanung steht die korrekte Identifikation des Indextumors sowie dessen Ausdehnung für die gezielte Biopsie. Entspricht der Gleason Score den Kriterien für die fokale HIFU-Therapie, kann die Behandlung anhand des mpMRT geplant werden. Die radikale Prostatektomie stellt die häufigste primäre Therapieoption im lokal begrenzten und fortgeschrittenen Prostatakarzinom dar. Angesichts der guten Korrelation zwischen dem mpMRT und Großflächenschnitten der Prostata bietet sich eine Nutzung für die Therapieplanung an. Die Radiotherapie wird in kurativer Intention ebenfalls als primäre Therapieoption eingesetzt. Weiterhin kann sie adjuvant bei entsprechender Risikokonstellation früh postoperativ in kurativer Intention erfolgen. Auch beim PSA-Rezidiv nach einer radikalen Prostatektomie steht die Salvage-Radiotherapie als kurative Therapie zur Verfügung und kann mit einer antihormoneller Behandlung kombiniert werden.
