Zusammenfassung

Insbesondere mehrfachverletzte Patienten erleiden in vielen Fällen auch ein Thoraxtrauma. In der ersten Phase haben die Identifikation und die Behandlung lebensbedrohlicher Verletzungen, bei denen der Spannungspneumothorax eine besonders wichtige Rolle einnimmt, absolute Priorität. Hierbei sollte der Advanced-Trauma-Life-Support(ATLS)-Algorithmus angewendet werden. In den meisten Fällen reicht zur Behandlung die Einlage einer Thoraxdrainage aus, bei einigen Verletzungen ist aber die Notfallthorakotomie als lebensrettender Eingriff unumgänglich. Im weiteren Verlauf sind insbesondere der instabile Thorax und der retinierte Hämatothorax für Komplikationen wie Lungenversagen und Thoraxempyem verantwortlich. Eine frühzeitige operative Intervention im Sinne einer Stabilisierung der Rippenfrakturen bzw. einer thorakoskopisch unterstützten Hämatomausräumung kann zur Vermeidung dieser Komplikationen beitragen.
