Tiefe Hirnstimulation bei hyperkinetischen Bewegungsstörungen

Zusammenfassung

Die neurologischen Bewegungsstörungen Dystonie, Tremor, Chorea und Tics werden als hyperkinetische Störungen zusammengefasst. Alle beruhen auf Netzwerkstörungen der Basalganglienschleife und treten z. T. isoliert oder in Kombination mit anderen neurologischen oder psychiatrischen Symptomen auf. Die tiefe Hirnstimulation (THS) stellt besonders in dieser medikamentös sehr eingeschränkt therapiefähigen Gruppe eine wichtige Erweiterung der therapeutischen Optionen dar. Hierbei werden Elektroden invasiv in unterschiedliche subkortikale Hirnareale implantiert, um durch hochfrequente Stimulation eine Besserung der motorischen Symptome zu erreichen. Als bereits etablierte Indikationen gelten die idiopathischen generalisierten und segmentalen Dystonien sowie der essenzielle Tremor, jedoch entwickeln sich durch das zunehmend bessere Verständnis der Systempathophysiologie immer mehr Ansätze zur Behandlung weiterer Erkrankungen dieses Formkreises. Dieser Artikel soll einen Überblick über häufige hyperkinetische Bewegungsstörungen unter dem Blickpunkt der jüngeren Entwicklungen der Anwendungsmöglichkeiten der THS geben.
