Chronischer Tinnitus aus psychologischer Sicht

Zusammenfassung

Chronischer Tinnitus ist ein häufiges, mitunter stark belastendes Phänomen, das in der chronischen Phase zuvorderst psychologisch aufrechterhalten wird. Hierbei attribuieren Betroffene Schwankungen ihrer Gefühlswelt oftmals kausal auf das Tinnitussymptom. Aus psychotherapeutischer Sicht sind jedoch sowohl das Symptomerleben als auch die emotionale Belastung Betroffener stattdessen von komplexen, dynamischen Wechselwirkungen zwischen (a) biographischen Erfahrungen, (b) Persönlichkeitseigenschaften, (c) der subjektiven Bewertung intrapsychischer und zwischenmenschlicher Reizkonstellationen, (d) Gefühlszuständen, und (e) intrapsychischen oder zwischenmenschlichen Bewältigungsstrategien geprägt. Nach einer kurzen Einführung verbindet die vorliegende Übersichtsarbeit eine etablierte psychologische Theorie zur Entstehung und Aufrechterhaltung des chronischen Tinnitus mit einem allgemeinen Vulnerabilitäts-Stress-Bewältigungsmodell. Diesbezüglich stellen wir kurz relevante Forschungsergebnisse aus somatischer und psychologischer Perspektive vor – und leiten schließlich Schlussfolgerungen zu psychotherapeutischen Präventions- und Behandlungsmaßnahmen ab.

Obstruktive Schlafapnoe: Das steht abseits von CPAP auf dem Plan

Die obstruktive Schlafapnoe kann durch verschiedene Verfahren behandelt werden wie Lagetherapie und chirurgische Interventionen. Eine medikamentöse Therapie kommt zwar (noch) nicht infrage. Auf dem Pneumologie-Kongress wurden aber erste vielversprechende Ansätze präsentiert.

Ein 43-jähriger Patient stellt sich wegen Anfällen von Drehschwindel in der Notaufnahme vor. Diese kämen immer nur dann, wenn er zuvor Musik über Kopfhörer gehört habe. Könnte ein Zusammenhang bestehen?

Weniger Bargeld, weniger Erstickungsnotfälle?

Dadurch, dass immer seltener mit Bargeld gezahlt wird, könnte die Rate an Erstickungsnotfällen bei Kindern zurückgehen. Dieser Hypothese ist ein britisches Forschungsteam in Klinikdaten aus den letzten zweieinhalb Jahrzehnten nachgegangen.

Wer schlechter riechen kann, stirbt früher

Ein eingeschränkter Geruchssinn scheint bei Älteren mit einer höheren Gesamtsterblichkeit einherzugehen. Dafür sprechen Längsschnittdaten aus Stockholm. Besonders bedeutsam sind dabei wohl Zusammenhänge mit Gebrechlichkeit und Neurodegeneration.
