Tomographic Analysis of the Pterygomaxillary Region in Atrophic Maxilla for the Placement of Pterygoid Implants

Abstract
Introduction
The objective of this study was to determine the anatomical and technical parameters for the installation of dental implants in the pterygomaxillary region.
Materials and Method
This is a cross-sectional imaging study. Twenty bilateral computed tomography scans (40 regions) were analyzed using the ImplantViewer program and subsequently tabulated in Microsoft Excel for statistical analysis in the GraphPad program with a 95% confidence level.
Results
The average implant length ranged from 14 to 18 mm, with an average of 16.2 mm (1.281 right and 16.05 left). The median density in the maxillary tuber region was 169 HU (211.7) (Hounsfield units), and in the pterygoid region, it was 559.55 HU (2.71.7) p-value < 0.0001. The average distance from the implant head to the floor of the maxillary sinus was 4.98 mm (3.058 right) and 4.41 mm (3.55 left), p-value 0.587. Regarding the pterygoid anatomical radiographic prediction (PARP classification), the most common was type 1, in 19 analyses, followed by type 4 in 7 analyses.
Conclusion
Pterygoid implants can be a safe and effective alternative for the rehabilitation of atrophic jaws. Tomographic analysis is essential to define the safe installation of implants in the pterygomaxillary region.
