Gastrointestinale Stromatumore (GIST) und Leiomyome sind im Vergleich zu den Karzinomen seltene, submukös wachsende Neoplasien. Beide Tumorarten sind verschiedene Entitäten, haben aber historisch eine gemeinsame Betrachtung, da die immunhistochemische Differenzierung der GIST von anderen Entitäten erst ab den 1980er-Jahren eindeutig möglich war. Dies ist bemerkenswert, da Leiomyome benigne Läsionen darstellen, jeder GIST jedoch als potenziell maligne einzustufen ist.
Die chirurgische komplette Resektion ist der Goldstandard in der Therapie von GIST. Die wichtigsten Aspekte der chirurgischen Therapie sind die Entfernung des gesamten Tumors mit negativen mikroskopischen Resektionsrändern (R0) und der Erhalt der Tumorkapsel. Eine Tumorruptur ist unbedingt zu vermeiden.
Durch die Therapie mit Tyrosinkinase-Hemmern hat sich die Prognose der GIST deutlich verbessert. Diese Entwicklung ist beispielhaft für die zielgerichtete Tumortherapie („targeted therapy“).
