Krankheiten des Mittelohrs bei Kindern und Jugendlichen

Erkrankungen des Mittelohres sind häufig, sie sind meist entzündlicher Genese, Fehlbildungen hingegen treten selten auf. Oft entsteht durch Tubenventilationsstörungen und adenoide Vegetationen ein Sero- oder Mukotympanon, das meist einer operativen Behandlung in Form einer Adenotomie und einer Paukendrainage bedarf. Als weitere Folgen können eitrige Mittelohrentzündungen auftreten, die unter antibiotischer Therapie relativ schnell ausheilen. In seltenen Fällen kann sich daraus eine Mastoiditis entwickeln, die meist operativ, selten konservativ behandelt wird. Als weitere Folge der chronischen Belüftungsstörung können Cholesteatome entstehen, die grundsätzlich operativ behandelt werden müssen. Verletzungen des Mittelohres können direkt z. B. durch spitze Gegenstände oder indirekt z. B. durch enorme Druckerhöhung im Gehörgang hervorgerufen werden. Sie treten häufig im Rahmen von Längsfrakturen des Felsenbeins auf. Neben der Perforation des Trommelfells kann es auch zu einer Luxation der Gehörknöchelchenkette kommen, die durch eine Tympanoplastik behandelt wird.
