Rektumkarzinom: Indikationen zur neoadjuvanten und adjuvanten Therapie

Die neoadjuvante Radiochemotherapie war viele Jahre Standard für das Rektumkarzinom in den UICC-Stadien II und III. In den letzten Jahren konnte gezeigt werden, dass durch MRT-basierte Chirurgie für Patienten mit negativem zirkumferenziellen Resektionsrand durch alleinige Chirurgie geringe Lokalrezidivraten und sehr gute onkologische Ergebnisse erzielt werden können. Die „totale neoadjuvante Therapie“ ergänzt die präoperative Radio- oder Radiochemotherapie um eine zusätzliche präoperative Systemtherapie. Durch die „totale neoadjuvante Therapie“ konnten höhere Raten an pathologischer Komplettremission und ein verbessertes onkologisches Outcome im Vergleich zur bisherigen neoadjuvanten Radiochemotherapie erreicht werden. Bei klinischer Vollremission nach Neoadjuvanz bleibt die radikale Resektion mit totaler mesorektaler Exzision der Standard. Ob in diesem Szenario das „Watch-and-wait-Konzept“ im onkologischen Outcome gleichwertig ist, sollte in prospektiv-randomisierten Phase-III-Studien evaluiert werden.
