Topische Schmerztherapie bei oraler Mukositis

Zusammenfassung

Hintergrund

Patienten unter Chemo- oder Radiotherapie beschreiben den Mukositisschmerz als das quälendste Symptom der Therapie, in dessen Folge häufig eine inadäquate Nahrungsaufnahme, Mangelernährung und sogar Therapieverschiebungen und -abbrüche auftreten. Für die behandelnden Ärzte stellt die adäquate Schmerztherapie eine erhebliche Herausforderung dar. Ziel dieser Arbeit ist es, einen Überblick über die bisher in Studien verwendeten Substanzen zur topischen analgetischen Behandlung der Mukositis zu geben.

Methoden

Eine systematische Suche in PubMed wurde mit einer Kombination der Termini „mucositis“ „pain“ und „topical“ oder „local“ durchgeführt. Zusätzlich wurden die Literaturlisten der gefundenen Publikationen auf geeignete Sekundärliteratur durchsucht.

Ergebnisse

Insgesamt wurden 47 Arbeiten eingeschlossen. Die Evidenz zur topischen Schmerztherapie der Mukositis ist gering. Die meisten hochwertigen Studien gibt es zur Anwendung von Opioiden, Kortikoiden und Benzydamin. Für die restlichen Substanzen gibt es nur wenige, wissenschaftlich schlechte und teils widersprüchliche Berichte.

Schlussfolgerung

Zusammenfassend kann man sagen, dass die topische Schmerztherapie der Mukositis derzeit mehr auf Empirie als auf gut designten, methodisch hochwertigen Studien basiert.
