Surgical site infection after hernia repair with a synthetic prosthesis is a feared complication for all surgeons. In this setting, we hope infection will be “only” superficial and the mesh will not get involved. When purulent discharge through the wound persists and the patient comes back at 2 or 3 months (sometimes 2 or 3 years), some surgeons act like ostriches, evoke a seroma or stitch rejection, and continue to hope… Nevertheless, most surgeons are concerned in this setting by the possibility of salvage of the mesh with local therapy or the further need for mesh removal. …
