Zusammenfassung

Die transösophageale Echokardiographie (TEE) ist in der Kardiochirurgie zur Diagnosestellung sowie als hämodynamisches Monitoring- und „guiding tool“ fest etabliert. Dynamische und (patho)physiologische Prozesse des Herzens können unmittelbar dargestellt werden. Im Idealfall lassen sich Therapieänderungen ableiten. Daher wird die TEE zunehmend auch bei nichtkardiochirurgischen Risikoeingriffen und im interventionellen Setting angewendet. Im 1. Teil dieser Weiterbildungsreihe werden allgemeine Aspekte zum Ablauf der TEE-Untersuchung sowie ihre Indikationen und Kontraindikationen dargestellt. Klinische Einsatzbereiche, in denen die TEE eine Rolle im perioperativen Monitoring spielen kann, werden skizziert. Der 2. Teil fokussiert einen Notfalluntersuchungsgang und Differenzialdiagnosen, die bei intraoperativer hämodynamischer Instabilität oder ungeklärter Hypoxämie mithilfe der TEE gestellt werden können. Ein Ausblick auf die Nutzung einer computergestützten TEE-Auswertung schließt den Beitrag ab.
