Tumor Budding beim kolorektalen Karzinom: Konsensus für den klinischen Einsatz

Zusammenfassung

Hintergrund

Tumor Budding beim kolorektalen Karzinom wurde in der Literatur als robuster Biomarker identifiziert. Dieses morphologische Phänomen, nämlich Tumoreinzelzellen und kleine Tumorzellgruppen, die sich von der Haupttumormasse lösen, kann auch in gewissen therapierelevanten Situationen genutzt werden. Noch wird Tumor Budding allerdings aufgrund einer mangelnden standardisierten Auswertungsmethode kaum in der Praxis eingesetzt. Die Internationale Tumor Budding Konsensus Konferenz (ITBCC), die 2016 in Bern stattfand, hat evidenzbasierte Richtlinien zur Auswertung dieses Biomarkers verfasst.

Ziel der Arbeit

Darstellung des aktuellen Wissensstands über Tumor Budding mit Schwerpunkt auf klinisch relevanten Einsatzmöglichkeiten.

Material und Methoden

Auswertung und Zusammenfassung der relevanten Literatur und Darstellung der Evidenzlage als Basis für die ITBCC.

Resultate

Heutige Erkenntnisse sprechen dafür, dass Tumor Budding dem morphologischen Korrelat der epithelialen mesenchymalen Transition entspricht. Die starken Assoziationen mit Lymphknotenmetastasen und schlechterem Überleben haben zur Folge, dass Tumor Budding beim endoskopisch resezierten pT1 und kolorektalen Karzinom Stadium II das Patientenmanagement beeinflussen kann (onkologische Segmentresektion bzw. Diskussion adjuvante Chemotherapie). Gemäß ITBCC soll Tumor Budding in diesen beiden Szenarien angegeben werden.

Schlussfolgerung

Die Richtlinien der ITBCC bilden eine Grundlage für die Aufnahme von Tumor Budding in internationale Richtlinien. Somit ist zu erwarten, dass dieser Marker zunehmend rapportiert wird. Es wird empfohlen, Tumor Budding im Rahmen von interdisziplinären Konferenzen als zusätzlichen Parameter zu berücksichtigen.

Bei Immuntherapien das erhöhte Thromboserisiko beachten
