Zusammenfassung

Das Urothelkarzinom des oberen Harntrakts (UTUC) ist eine seltene Entität. Die Inzidenz liegt in Deutschland derzeit bei etwa 2/100.000 mit einem Verhältnis zwischen Frauen und Männern von 1:2,5. Die meisten klinischen Zeichen sind unspezifisch und unergiebig, weswegen eine frühzeitige Diagnosestellung nur selten gelingt. Die Computertomographie mit Kontrastmittel und Urographiephase bildet gemeinsam mit der diagnostischen Ureterorenoskopie aktuell den Goldstandard in der Diagnostik des UTUC. Bei der operativen Behandlung des UTUC bleibt derzeit die radikale Nephroureterektomie (RNU) unter Mitnahme einer Blasenmanschette das Mittel der Wahl, obgleich sich das radikale Vorgehen hin zu laparoskopischen/robotischen oder endourologischen Verfahren unter Erhalt der Niere entwickelt. Aufgrund der hohen Rezidivrate (22–47 %) eines Urothelkarzinoms in der Harnblase sind engmaschige Verlaufskontrollen nach RNU obligat.
