Standardtherapie von Plattenepithelkarzinomen der Mundhöhle gemäß Leitlinien

Zusammenfassung

Das chirurgische Vorgehen beim Plattenepithelkarzinom der Mundschleimhaut ist, die Operabilität des Patienten vorausgesetzt, die tragende Therapiesäule mit stadienabhängiger adjuvanter Radiotherapie und (Radio)chemotherapie. Obwohl es viele offene Fragen gibt, ist die Datenlage zur Effektivität der operativen Behandlung insgesamt gut, sowohl was die Resektion des Primärtumors als auch die Behandlung der abführenden Lymphwege angeht. Entscheidender Prognosefaktor ist neben dem Lymphknotenstatus der erreichte Sicherheitsabstand und Resektionsstatus, womit der Qualität der chirurgischen Resektion die wesentliche Bedeutung zukommt. Auch zur Frage der Unterkieferresektion liegen Daten vor, wobei hier Aussagen auf hohem Evidenzniveau schwieriger sind. Für die niedrigen bis mittleren Tumorstadien ist die operative Behandlung auch hinsichtlich der posttherapeutischen Funktionalität und Lebensqualität der Maßstab.
