(Wieder) eine Revolution in der Therapie des benignen Prostatasyndroms? Aquaablation und Prostataembolisation

Zusammenfassung

Die operative Therapie des benignen Prostatasyndroms (BPS) ist eine der häufigsten Eingriffe der Urologie. In den letzten Jahren haben sich zahlreiche alternative Verfahren entwickelt, welche das Ziel haben, die potenziellen Limitationen der transurethrale Resektion der Prostata (TURP) bei vergleichbarer Wirksamkeit und geringerer Morbidität zu überwinden. Das AquaBeam®-System zeigte in einer randomisierten Studie verglichen zur TURP, zu welcher mittlerweile 3‑Jahre-Daten vorliegen, eine geringere Rate an unerwünschten Ereignissen und vergleichbare funktionelle Ergebnisse. Auch in einer Kohortenstudie an Patienten mit Prostatavolumina bis 150 ml zeigten sich suffiziente funktionelle Resultate, wobei die Rate an Transfusionen die Frage nach der optimalen Koagulation aufwirft. Weitere Kohortenstudien mit kurzfristigem Follow-up konnten die Resultate der randomisierten Studien bestätigen. Die Embolisation der Prostataarterien (PAE) wurde in mehreren randomisierten Studien mit der TURP verglichen. In randomisierten Vergleichsstudien ist die Reduktion BPS-assoziierter Symptome nach PAE der nach einer TURP nur leicht unterlegen. Im Gegensatz hierzu verbessern sich funktionelle Parameter nach Embolisation deutlich schlechter als nach TURP, was an der unterlegenen Desobstruktion zu liegen scheint. Zudem fehlen bisher Langzeitresultate aus klinischen Studien, so dass bezüglich der Haltbarkeit der Desobstruktion bisher keine abschließende Beurteilung möglich ist. Zusammenfassend handelt es sich bei beiden diskutierten Verfahren um potenzielle Alternativen zu Standardverfahren, welche bei adäquater Patientenselektion das operative Armamentarium im Sinne einer individualisierten Therapie des BPS sinnvoll ergänzen können.
