Zusammenfassung

Hintergrund

Diabetes mellitus Typ 2 ist eine komplexe Erkrankung, die mit einem erhöhten Risiko für mikro- und makrovaskuläre Erkrankungen verknüpft ist. Die Bedeutung einer verbesserten metabolischen Kontrolle und insbesondere postprandialer Blutzuckerexkursionen wird hierbei kontrovers diskutiert.

Datenlage

Während die Resultate der Mehrzahl der epidemiologischen Untersuchungen auf einen Zusammenhang zwischen postprandialen Blutzuckerspitzen und dem Auftreten vaskulärer Komplikationen bei Personen mit und ohne Diabetes hinweisen, erscheint die Datenlage aus prospektiven, randomisierten klinischen Studien unklar. In zahlreichen mechanistischen Arbeiten konnte eine Verbindung zwischen akuten Blutzuckerspitzen und der Aktivierung vasopathogener Signalkaskaden dargestellt werden.

Resümee

Ziel dieses Beitrags ist, einen Überblick über die derzeit verfügbare Evidenz zur Bedeutung der Einstellung des postprandialen Blutzuckerspiegels für eine Risikoreduktion vaskulärer Komplikationen bei Patienten mit Diabetes mellitus Typ 2 zu geben.
