Botulinumtoxin bei überaktiver Blase

Zusammenfassung

Die idiopathische überaktive Blase (OAB) besteht aus einem Symptomenkomplex aus imperativem Harndrang mit oder ohne Inkontinenz, Nykturie und erhöhter Miktionsfrequenz ohne Vorliegen einer Infektion oder sonstige Pathologien, insbesondere ohne Vorliegen einer neurologischen Erkrankung, und ist eine Ausschlussdiagnose. Führen die konservativen Maßnahmen mittels Beckenboden- und Verhaltenstherapie nicht zum gewünschten Erfolg, wird gemäß der OAB-Stufentherapie zusätzlich eine medikamentöse Therapie empfohlen. Bei einer therapierefraktären OAB wird laut aktuellen Leitlinien Onabotulinumtoxin A mit einem Empfehlungsgrad A empfohlen. Die zugelassene Dosis sind 100 E Botulinumtoxin, welche transurethral in den Detrusor vesicae injiziert werden. Der Therapieeffekt hält im Mittel 6 bis 9 Monate an, und die Injektion kann unbegrenzt wiederholt werden. Aufgrund der geringen Komplikationsrate bei guter Erfolgsquote sowie der geringen Invasivität bietet Botulinumtoxin eine gute Behandlungsalternative bei der therapierefraktären OAB.
