Zusammenfassung

Hintergrund

Perkutane lokal ablative Therapieverfahren sind etablierte Verfahren in der Onkologie und sowohl in nationalen als auch in internationalen Leitlinien verankert.

Fragestellung

Es werden die verschiedenen lokal ablativen Verfahren und ihre Bedeutung in der Behandlung primärer und sekundärer maligner Leberraumforderungen dargestellt.

Material und Methoden

Die vorhandene Literatur und Leitlinienempfehlungen werden ausgewertet und diskutiert.

Ergebnisse

Die Radiofrequenz- und Mikrowellenablation haben als thermische Ablationsverfahren in den letzten Jahren die chemischen Verfahren weitgehend abgelöst. Ihr Einsatz erfolgt in der Regel ultraschall- oder computertomographiegesteuert. Mithilfe der kontrastverstärkten Sonographie lassen sich auch im B‑Bild schlecht abgrenzbare Läsionen sicher abladieren. Aufgrund des zugrunde liegenden physikalischen Wirkprinzips ist die lokale Ablation in der Leber in der Regel auf Raumforderungen mit einer Größe bis zu 3–5 cm limitiert. In der Behandlung des hepatozellulären Karzinoms erfolgt der Einsatz der Verfahren insbesondere in den sehr frühen oder frühen Stadien als Alternative zur Resektion oder als Überbrückungsmaßnahme zur Lebertransplantation. Weiterhin kann der Einsatz der lokal ablativen Verfahren bei selektionierten Patienten mit Lebermetastasen eines kolorektalen Karzinoms, eines Mammakarzinoms oder auch eines neuroendokrinen Tumors erfolgen.

Schlussfolgerung

Lokal ablative Verfahren haben einen großen Stellenwert in der Behandlung maligner Leberläsionen. Etablierte Verfahren sind sowohl die Radiofrequenzablation als auch die Mikrowellenablation, die beide auch ultraschallgesteuert eingesetzt werden können und mit einer geringen Komplikationsrate einhergehen.

Bei Immuntherapien das erhöhte Thromboserisiko beachten

Unter modernen Systemtherapien versechsfacht sich das VTE-Risiko. Warum diese Daten relevant für die Behandlung krebsassoziierter Thrombosen sind, erläutert Prof. F. Langer im Interview.
