An 8-year-old healthy girl presented with complaints of vomiting and fever, which was present for the previous 3 days and reached a maximum of 40 °C. The patient had manifestations of toxicity and left costovertebral angle tenderness on physical examination. Laboratory tests revealed leukocytosis (19,600/mm3); elevation in acute phase reactants (C-reactive protein (CRP) 369 mg/dl (0–5)), procalcitonin 3.4 ng/ml (0–0.1); pH 5.5; and protein +1, erythrocyte +1, leukocyte +2, and nitrite test positivity.
