Uveitis bei juveniler idiopathischer Arthritis

Zusammenfassung

Hintergrund

Die Uveitis ist die häufigste extraartikuläre Manifestation der juvenilen idiopathischen Arthritis (JIA). Nach Registerdaten aus Deutschland tritt sie bei etwa 12 % der Patienten auf und kann bei verzögerter Diagnose und inadäquater Therapie zur Sehminderung führen.

Fragestellung

Es erfolgt die Darstellung aktueller Aspekte von Diagnostik und Therapie.

Material und Methoden

Es handelt sich um eine Übersichtsarbeit.

Ergebnisse

Das Uveitisrisiko ist besonders hoch bei oligoartikulärem Verlauf, ANA (antinukleäre Antikörper)-Positivität und jungem Alter bei Auftreten der JIA. Während des oft asymptomatischen Uveitisverlaufes entwickeln sich häufig sehmindernde Komplikationen (wie Katarakt, Glaukom, Makulaödem). Voraussetzung für eine gute Visusprognose sind der frühzeitige Uveitisnachweis und eine interdisziplinäre Betreuung durch Augenärzte (an Uveitiszentren) sowie Kinder- und Jugendrheumatologen. Initial werden topische Kortikosteroide am betroffenen Auge verwendet. Bei unzureichender Wirksamkeit oder visusgefährdendem Verlauf tragen konventionelle synthetische (insbesondere Methotrexat) und biologische DMARDs („disease modifying antirheumatic drugs“) wesentlich zur Verbesserung des Langzeitverlaufes bei. Hierbei existiert die größte Evidenz für den Einsatz von Adalimumab. Alternativ finden auch Infliximab, Golimumab, Tocilizumab, Abatacept oder Rituximab Anwendung. Auch unter der DMARD-Therapie persistieren JIA und Uveitis häufig ins Erwachsenenalter. Bedeutungsvoll für die Langzeitprognose und die Lebensqualität der Patienten sind die Prophylaxe, der frühzeitige Nachweis und das adäquate Management der Uveitis und von uveitisbedingten Augenkomplikationen (ggf. auch operativ).

Schlussfolgerungen

Bei frühzeitigem Nachweis der JIA-assoziierten Uveitis und unverzüglicher und effektiver insbesondere kortisonsparender DMARD-Therapie kann oft eine Inaktivität der Uveitis erzielt und eine Sehminderung vermieden werden.
