Leitlinie Sklerosierungsbehandlung der Varikose

Bei der Sklerosierungsbehandlung handelt es sich um die gezielte chemische Ablation von Varizen durch die intravenöse Injektion eines flüssigen oder aufgeschäumten Sklerosierungsmittels [1, 2]. Die zu behandelnden Venen können intradermal, subkutan und/oder transfaszial (Perforansvenen) sowie als epi-, supra- und subfasziale Gefäße bei venösen Malformationen vorliegen. Das Sklerosierungsmittel zerstört das venöse Endothel und evtl. weitere Bereiche der Venenwand und wird durch Blutbestandteile und zirkulierende Zellen inaktiviert [1, 2]. Nach erfolgreicher Sklerosierung kommt es längerfristig zur Umwandlung der Varizen in einen bindegewebigen Strang, einem als Sklerose bekannten Prozess [36]. Ziel der Sklerosierungsbehandlung ist nicht die Thrombosierung der Venen, da die behandelten Venen rekanalisieren könnten, sondern die definitive Transformation der thrombosierten Vene in einen Bindegewebsstrang. Dieser kann nicht rekanalisieren und entspricht in seinem funktionellen Ergebnis der operativen Entfernung einer Varize oder der endovenösen thermischen Ablation. …
