Therapie des Herpes zoster und der postherpetischen Neuralgie

Zusammenfassung

Herpes zoster (HZ) wird durch die Reaktivierung des Varicella-zoster-Virus (VZV) ausgelöst. Die Inzidenz von HZ und assoziierten Problemen nimmt mit steigendem Lebensalter zu. Mit einer lebenslangen Prävalenz von 30 % erkrankt jeder zweite 85-Jährige sozusagen 1‑mal an einem HZ. Die 3 Therapiesäulen umfassen antivirale, topische und analgetische Therapien. Eine extreme Belastung kann der akute und persistierende Schmerz, der dann in eine postherpetische Neuralgie (PHN) übergeht, darstellen. Therapeutisch herausfordernd kann akut und im Verlauf der neuropathische Schmerz sein. Eine wesentliche Besserung im Sinne der Prävention eines HZ und zudem der Reduktion des Kollektivs erkrankter Patienten, die eine PHN entwickeln, stellt die Impfung mit HZ-Impfstoffen dar. Von der ständigen Impfkommission des Robert Koch-Instituts (STIKO) wird ein Totimpfstoff bei allen Personen ab dem 60. Lebensjahr empfohlen. Risikogruppen wie Patienten mit Immunschwäche wird die Impfung bereits ab einem Alter von 50 Jahren nahegelegt.
