Varizenblutung bei Patienten mit Leberzirrhose

Zusammenfassung

Bei der Leberzirrhose entstehen aufgrund des portalen Hypertonus Kollateralen, die zur intestinalen Blutung führen können. Eine Blutung ereignet sich jedoch nicht, wenn der Lebervenenverschlussdruck <12 mmHg ist (Norm ≤6). Der portale Hypertonus entsteht auf der einen Seite durch die Erhöhung des Ausflusswiderstandes (Umbau der Leber), auf der anderen Seite durch einen vermehrten Blutfluss in das Portalsystem. Die Therapie setzt einerseits systemisch an beiden pathogenetischen Mechanismen (medikamentöse Portaldrucksenkung und Shunts) an, andererseits lokal an der Blutungsquelle selbst (endoskopische Interventionen). Hierbei werden 4 Ziele verfolgt: 1. Verhinderung der Ausbildung von Varizen; 2. Verhinderung der Blutung aus bereits entstandenen Varizen; 3. Behandlung der akuten Blutung; 4. Verhinderung der Rezidivblutung. Je nach Situation wird eher eine systemisch medikamentöse oder interventionelle (Endoskopie, Shunt) Therapie gewählt, wobei die Verfahren auch kombiniert werden.
