Reizdarm bei Kindern und Jugendlichen

Zusammenfassung

Bei Schulkindern mit chronisch rezidivierenden Bauchschmerzen liegt in vielen Fällen ein Reizdarmsyndrom (RDS) vor. Genetische, familiäre Faktoren, frühe somatische Traumata und gastrointestinale Infektionen führen hierbei u. a. zu intestinaler Inflammation, vermehrter Darmpermeabilität und Motilitätsveränderungen. Diese bedingen eine viszerale Hypersensitivität, die durch psychische und zentrale Faktoren moduliert wird. Häufige organische Differenzialdiagnosen wie Kohlenhydratmalabsorptionen, Infektionen, Obstipation, Zöliakie und chronisch entzündliche Darmerkrankungen sind zu berücksichtigen. Diagnostisch wichtig sind Anamnese, klinische Untersuchung, eine Basislabordiagnostik und der Ausschluss von Kohlenhydratmalabsorptionen. Invasive Untersuchungen sind zu vermeiden. Die Therapie beginnt mit Beratung über das biopsychosoziale Bauchschmerzmodell. Kognitiv-verhaltenstherapeutische Interventionen und Hypnotherapie sind langfristig wirksam. Effekte medikamentöser Optionen (abgesehen von Macrogol bei Obstipation) und ungezielter diätetischer Interventionen sind nicht evidenzbasiert.
