Verhinderung der postnatalen Zytomegalievirusinfektion bei Frühgeborenen

Zusammenfassung

Hintergrund

Zytomegalievirus(CMV)-IgG(Immunglobulin G)-seropositive Mütter von Frühgeborenen (FG) mit einem Geburtsgewicht <1500 g oder Gestationsalter <32 Schwangerschaftswochen reaktivieren in bis zu 96 % CMV isoliert in ihrer Brustdrüse und sezernieren es in die Muttermilch (MM).

Neonatologischer Verlauf

Ungefähr ein Drittel der FG infiziert sich. Etwa die Hälfte zeigt mindestens 1 Symptom und hat das eindeutig abgrenzbare Krankheitsbild der symptomatischen postnatalen CMV(pCMV)-Infektion. Die meisten Infektionen verlaufen selbstlimitierend, allerdings sind schwere und sogar letale Verläufe beschrieben, auch das Risiko für bronchopulmonale Dysplasie scheint erhöht. Evidenz für oder gegen eine antivirale Behandlung gibt es bislang nicht.

Langzeitverlauf

Bei Langzeituntersuchungen zum kognitiven Outcome im Kindes- und Jugendalter gibt es reduzierte Scores auf der Wechsler Intelligence Scale und magnetresonanztomographische Hinweise auf negative Einflüsse einer pCMV-Infektion auf ehemals sehr kleine Frühgeborene, die sich erst im Lauf der Entwicklung manifestieren. Eine höhere Inzidenz an Hörstörungen wurde bislang nicht beobachtet.

Vermeidung der Zytomegalievirusinfektion

Methoden zur sicheren Vermeidung der pCMV-Infektion beschränken sich auf die Hitzeinaktivierung der MM, davon sind kommerziell die Holder-Pasteurisierung (62,5 °C, 30 min) und die Kurzzeithitzeinaktivierung (62 °C, 5 s) verfügbar. Beide sind sicher viruzid, hinsichtlich der Denaturierung bioaktiver Moleküle ist die Letztere schonender. Beim klinischen Einsatz dieser Kurzzeitmethode konnte die Inzidenz der pCMV-Infektion sicher gesenkt werden. Die Kryoinaktivierung (−20 °C) ist nicht sicher viruzid.
