Indikationen und pathologiespezifische rekonstruktive Verfahren

Zusammenfassung

Risse der Rotatorenmanschette (RM) reichen von Partialrupturen einzelner Sehnen über transmurale Rupturen bis hin zu RM-Massenrupturen. Sie können sowohl als Folge eines einmaligen Traumas oder auf der Basis degenerativer Prozesse entstehen. Ihre Behandlung erfordert daher eine gezielte Diagnostik und ein differenziertes Vorgehen, wobei der Wahl des Therapieverfahrens pathologie- und patientenspezifische Faktoren zugrunde gelegt werden. Pathogenese, Rupturmorphologie, klinische Symptomatik und der funktionelle Anspruch des Patienten bestimmen das therapeutische Vorgehen. Bei jungen Patienten und bei hohen funktionellen Anforderungen ist eine frühzeitige operative Naht der rupturierten RM auch aufgrund des hohen Risikos einer Befundprogredienz zu erwägen. Die Ergebnisse der RM-Naht korrelieren negativ mit der Rupturgröße, dem Retraktionsgrad, den muskulären Veränderungen und dem Patientenalter. Dieser Beitrag gibt eine Übersicht über die verschiedenen Pathologien und die Indikationsstellung sowie über moderne pathologiespezifische rekonstruktive Verfahren.
