Zusammenfassung

Hintergrund

Knorpeldefekte im Bereich der Hauptbelastungszonen des Kniegelenks sind häufig. Am ehesten sind der mediale Femurkondylus und die retropatellare Knorpelfläche betroffen. Die Unterscheidung von umschriebenen und gut abgegrenzten Knorpeldefekten von großflächigen degenerativen Knorpelläsionen ist sowohl hinsichtlich der Prognose als auch der chirurgischen Therapie von großer Bedeutung. Um knorpelreparativ chirurgisch erfolgreich zu sein, ist es unerlässlich, Alter, Größe, Position, Pathomechanismus des Knorpeldefekts, Aktivitätslevel und Erwartungen des Patienten in den Therapiealgorithmus mit einzubeziehen. Wichtige Kofaktoren wie Stabilität, Beinachse und Muskelbalance sollten ebenfalls berücksichtigt werden.

Ziel

Der Beitrag zeigt die aktuellen Verfahren und deren Ergebnisse zur Knorpelreparation im Bereich des Kniegelenks auf. Zudem soll dargestellt werden, dass eine präzise und umfassende präoperative klinische und radiologische Diagnostik zur Abklärung weiterer Pathologien unbedingt notwendig ist, da diese zur erfolgreichen Reparatur des Knorpeldefektes ebenfalls behandelt werden müssen.

Ergebnisse und Zusammenfassung

Zur präoperativen Planung knorpelreparativer Techniken sollten Röntgenbilder und eine Magnetresonanztomografie(MRT)-Untersuchung erfolgen. Bei mangelnder MRT-Verfügbarkeit wäre ein Arthro-Computertomogramm eine Alternative. Moderne und routinemäßig angewendete Verfahren zur Knorpelreparation am Kniegelenk sind die Mikrofrakturierung, die autologe matrixinduzierte Chondrogenese, die autologe Chondrozytentransplantation, die matrixassoziierte autologe Chondrozytenimplantation und die osteochondrale Transplantation. Erfolgreiche chirurgische knorpelreparative Maßnahmen erfordern neben einer korrekten und individualisierten Indikationsstellung die Adressierung von Kopathologien und eine standardisierte, der chirurgischen Vorgehensweise angepasste Rehabilitation. Evidenzbasierte Kriterien für einen genauen Zeitpunkt für die Rückkehr zu verschiedenen Sportarten nach individuell divergierenden operativen knorpelreparativen Maßnahmen existieren aktuell nicht.
