Vermeidung von Implantat-Fehlpositionierungen

Zusammenfassung

Hintergrund

Dargestellt wird die klinische Relevanz von Implantat-Fehlpositionierungen und deren Vermeidung.

Zielsetzung

Ziel war die Entwicklung von Strategien zur Vermeidung von Fehlpositionierungen von Beginn an.

Material und Methoden

Es erfolgte eine Literaturrecherche über Cochrane und Medline mit den Suchtermini vertical positioning of dental implants“, „horizontal positioning of dental implants“, „inter-implant distances“, „tooth-to-implant distance“, „distances of dental implants and neighbouring structures“, „implant failure“, „implants and biomechanical failure“, „distribution of implants“. Anhand der Abstracts wurden die Studien gesichtet und irrelevante Studien ausgeschlossen. Für die übrigen Studien wurde die Volltextversion analysiert und je Themenbereich im Vergleich betrachtet.

Ergebnisse

Im Themengebiet konnten nur wenige randomisierte klinische Studien erfasst werden. Die Ergebnisse werden unter den Aspekten präimplantologische Risikoeinschätzung, dreidimensionale Ausrichtung des Implantats, Angabe der Körnungsmindestdistanzen, Implantatneigung sowie Abstände zu Nachbarstrukturen erläutert.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
