Zusammenfassung

Die Katheterablation durch die Isolation der Pulmonalvenen (PVI) ist Standardtherapie bei Patienten mit symptomatischem therapierefraktärem Vorhofflimmern (VHF). Laut den aktuellen VHF-Leitlinien kann die PVI auch als Erstlinientherapie durchgeführt werden. In der CASTLE-AF-Studie wurden VHF-Patienten mit stark eingeschränkter linksventrikulärer (LV) Funktion in einem randomisierten Design einer Katheterablation oder einer medikamentösen Therapie unterzogen. Durch die Ablation wurde die Gesamtmortalität und die Hospitalisierungsrate in dieser Studie signifikant gesenkt. Dieses Ergebnis wird unterstützt durch die CAMERA-MRI-Studie. Der Nutzen der Katheterablation bei Patienten mit Herzinsuffizienz konnte in der großen randomisierten CABANA-Studie nicht reproduziert werden. Aufgrund einer hohen Cross-over-Rate ist dieses Ergebnis derzeit Bestandteil des wissenschaftlichen Diskurses, und die Konsequenzen für die klinische Praxis bleiben derzeit unklar. In der DECAAF-Studie wurde eine positive Korrelation von linksatrialer Fibrose und dem Risiko eines Wiederauftretens des VHF nach PVI beschrieben. Ob diese Areale bereits Teil in die initiale Ablationsstrategie miteinbezogen werden sollen, ist Bestandteil der laufenden DECAAF-II-Studie. Die Ergebnisse könnten sich sowohl auf die präprozedurale Planungsphase als auch auf zukünftige Ablationsstrategien auswirken. Schlussendlich gewinnt die High-„power short duration ablation“ (HPSD) zunehmendes wissenschaftliches Interesse. In der QDOT-Fast-Studie konnten durch den Einsatz eines neuen Katheters die Prozedur- und Durchleuchtungszeiten signifikant reduziert werden. Zukünftige Studien sind jedoch noch erforderlich, um die Langzeitergebnisse dieses neuartigen Ablationsansatzes zu bewerten.
