Vorkommen von Frequent Usern und Frequent Callern in einem großstädtischen Rettungsdienst: Indikatoren eines unzureichenden Gesundheits- und Sozialsystems?

Zusammenfassung

Hintergrund

Frequent User machen als solche Patienten, die wiederholt Notaufnahmen aufsuchen, einen nicht unerheblichen Anteil an den Gesamtpatienten aus. Häufig stehen psychosoziale Probleme im Vordergrund, zum Teil liegen aber auch somatische Erkrankungen vor.

Methode

Die Arbeit untersucht das Vorkommen von High und Super Frequent Usern im Berliner Rettungsdienst. Hierzu wurde ein Zeitraum von einem Jahr anhand der Daten des Einsatzleitrechners ausgewertet.

Ergebnisse

Es konnte gezeigt werden, dass 2,1 % aller Einsätze durch High Frequent User (7–10/Jahr) und Super Frequent User (≥11/Jahr) verursacht wurden. Ausgehend von 306.600 Einsätzen konnten 598 Personen als High und Super Frequent User identifiziert werden. High Frequent User verursachen im Durchschnitt 8 Einsätze pro Jahr, Super Frequent User 18 Einsätze pro Jahr. Exemplarisch konnte anhand eines Falls, bei der ein Super Frequent User 194 Einsätze pro Jahr verursachte, gezeigt werden, dass über ein individuelles Case-Management Einsatzzahlen reduziert werden können.

Schlussfolgerungen

Allgemein zeigen sich gerade in Großstädten Zusammenhänge zwischen der Sozialstruktur und dem Einsatzaufkommen. Verschiedene Ansätze aus dem Ausland haben gezeigt, dass ein spezielles individualisiertes Case-Management für diese spezielle Gruppe hilfreich ist und zur Reduzierung des Anrufaufkommens beitragen kann.
