Vorteil für PFO-Verschluss gegenüber medikamentöser Therapie

Erneutes Auftreten thromboembolischer Ereignisse

Neben dem persönlichen Schicksal eines jeden Patienten der einen Schlaganfall erleidet, spielen die Folgen eines Apoplex eine ausgeprägte sozioökonomische Rolle [1]. In 25 – 40% der Fälle bleibt die Ursache für einen Schlaganfall unklar, mit steigender Inzidenz des sog. kryptogenen Schlaganfalls insbesondere bei jüngeren Patienten [2, 3]. Aus epidemiologischen Studien ist bekannt, dass eine Assoziation von kryptogenen Schlaganfällen und dem Vorliegen eines persistierenden Foramen ovale (PFO) besteht [4, 5], welches in etwa 38% der Patienten mit kryptogenem Apoplex zu finden ist [6]. Als pathophysiologische Ursache des Schlaganfalls ist der Übertritt eines Thrombus / Embolus mit konsekutivem Auftreten einer „paradoxen Embolie“ weitgehend akzeptiert [7]. Der Verschluss dieser potenziellen Übertrittspforte ist eine reizvolle Option bei Patienten mit kryptogenem Schlaganfall und PFO. Auch wenn der interventionelle PFO-Verschluss heutzutage mit minimalem operativem Risiko durchgeführt werden kann und Beobachtungsstudien einen Vorteil für den PFO-Verschluss beim kryptogenen Schlaganfall zeigen [3, 8], konnten randomisierte Studien, die den kathetergestützten PFO-Verschlusses gegen eine rein medikamentöse Therapie verglichen, (bislang) keine eindeutige Überlegenheit zeigen: In der CLOSURE-I- (StarFlex® Device), in der RESPECT- und der PC-Studie (Amplatzer PFO Occluder) erfolgte ein randomisierter Vergleich zwischen PFO-Verschluss und medikamentöser Therapie (Acetylsalicylsäure, duale Plättchenhemmung oder Vitamin-K-Antagonisten) [9, 10, 11]. Nach einem relativ kurzen Beobachtungszeitraum (2–4,5 Jahre) zeigte sich in allen Studien kein Unterschied im primären Endpunkt. In den „as-treated“- und „per protocol“-Analysen jedoch, also bei Patienten, die tatsächlich einen PFO-Verschluss erhalten hatten, zeigte sich in der RESPECT-Studie ein signifikanter Vorteil für den PFO-Verschluss. Hierbei profitierten insbesondere die Subgruppen der Patienten mit relevantem Shuntvolumen und atrialem Septumaneurysma vom PFO-Verschluss. …
