Wann und wie sollen Tumorpatienten ernährt werden?

Zusammenfassung

Gewichtsverlust und Ernährungsprobleme sind bei Tumorpatienten häufig. Folgen sind eine erhöhte Morbidität und Mortalität sowie eine verminderte Lebensqualität. Daher sollte bei einem Gewichtsverlust von 5% und mehr vom gesunden Ausgangsgewicht bei jeder Konsultation eine Ernährungsdiagnostik mit Bestimmung der Energiezufuhr und bei Bedarf eine Ernährungsberatung erfolgen. Oral kann meist mit einer auf die Wünsche und Bedürfnisse des Patienten individuell abgestimmten Vollkost ernährt werden. Bei unzureichender (Energiezufuhr unter 60% des errechneten Bedarfs erwartet für mindestens 10 Tage) oder nicht möglicher (Energiezufuhr unter 500 kcal/Tag erwartet für mindestens 5 Tage) oraler Ernährung ist eine künstliche Ernährung indiziert, zunächst als enterale Ernährung. Die empfohlene Energiezufuhr liegt bei 25 kcal/kg/Tag für bettlägerige und bei 30–35 kcal/kg/Tag für mobile Patienten mit einem Fettanteil von über 35% der Gesamtenergiezufuhr, die Eiweißzufuhr bei 1,2–1,5 g/kg/Tag. Die Empfehlungen zur Vitamin- und Spurenelementzufuhr entspechen den Richtlinien der Fachgesellschaften. In diesem Beitrag wird auf spezielle Ernährungsempfehlungen prä- und postoperativ, unter Chemo-und Radiotherapie sowie außerhalb einer Tumortherapie eingegangen.
