Weichteilsarkome und gastrointestinale Stromatumoren

Zusammenfassung

Weichteilsarkome sind seltene Tumoren, die durch ihr vielgestaltiges klinisches Erscheinungsbild und die meist interdisziplinäre Therapiestrategie eine besondere Herausforderung darstellen. Goldstandard für die Behandlung von primär resektablen Weichteilsarkomen ist die Resektion im Gesunden. Bei metastasierten Weichteilsarkomen ist die systemische Therapie von entscheidender Bedeutung. Die wirksamsten Einzelsubstanzen sind Anthrazykline und Ifosfamid. Mit einer Kombinationstherapie werden meist höhere Ansprechraten erreicht, allerdings auf Kosten der Toxizität. Bei fortgeschrittenen oder metastasierten gastrointestinalen Stromatumoren ist Imatinib in einer Dosierung von 400 mg/Tag der Goldstandard. Bei Mutation im KIT-Exon 9 sollte eine Tagesdosis von 800 mg eingesetzt werden. Nach Versagen von Imatinib wird die Therapie mit Sunitinib fortgesetzt. Für die Drittlinientherapie steht mit Regorafenib eine wirksame Behandlung zur Verfügung.
