Carotid blowout syndrome (CBS) is a rare but CBS is a fatal complication in patients undergoing aggressive treatment for head and neck cancer. Mortality of CBS was reported up to 76% [
1]. As recent treatment for cervical malignant tumors has been improved, patients with cervical malignant tumors are able to have a longer life expectancy than before. Therefore the number of CBS patients may be increased. It already has been reported endovascular treatment for CBS was effective to save the patient’s life. Large number data showed an in-hospital mortality rate was 8.2% [
2] after endovascular treatment. However one of problems of endovascular treatment for CBS is acute ischemic stroke. Acute ischemic stroke rate was described as 2.3–10.7% [
